Contacting Medical Records Department

Request for Access to Protected Health Information: Complete this form to have a copy of your medical records sent to you or another party. Parents and guardians should use this form for their dependents. 

Authorization for Release of Information: Use this form to allow third parties to request a copy of an individual's medical information. The individual must sign this authorization.

Processing time

• Allow a minimum of two days for records stored on site.
• Some records may require two to four weeks. Call ahead to confirm the timeline.
