Becoming a Patient

What To Bring

  • Comfort Items: comfortable clothing, toiletries, and any assistive devices, such as glasses and hearing aids
  • Identification: A photo ID, such as a driver's license, passport, or state-issued ID.
  • Insurance Information: Bring all applicable insurance cards and any necessary authorization forms.
  • Medications: A complete list of your current medications, including prescriptions, over-the-counter medications, and supplements.
  • Emergency Contacts: Names and phone numbers of family or friends to contact during your stay.
  • Advance Directives: If applicable, provide a copy of your living will or durable power of attorney for health care.

What to Leave at Home

  • Jewelry or other valuables
  • Luggage and other large personal belongings

Important Guidelines

Please keep the following in mind prior to your admission:

  1. Arrange for someone to drive you home after a procedure or an overnight stay.
  2. Your doctor and nurse will decide when you are ready to go home and will provide as much notice as possible for planning.
