When a patient needs a copy of their Barnes-Jewish West County Hospital medical record, the Health Information Management department is available to help.



The department hours are Monday through Friday 8 a.m.–4:30 p.m.

Request for Access to Protected Health Information:

To request yours or your child's medical records, please click on the link below to request a copy of your medical records. Please complete this form to have a copy of your medical records sent to you or to someone other than yourself. Note: Parents and guardians, please use this form for your patients.
