Shoulder Surgery: More Options, Longer-Lasting Results

At Barnes-Jewish Hospital, WashU Medicine orthopedic surgeon, Jay Keener, MD, and his team are using the latest technology and personalized implants to make shoulder replacement surgery more advanced. Through expert planning and innovative surgical techniques, they’re helping patients get back to the activities they love, with less pain and better long-term results.
A personalized plan for every shoulder
One significant improvement in shoulder replacement surgery is the ability to more precisely plan everything ahead of time, before making a single incision. Using 3D imaging and planning software, Dr. Keener and his team can see the patient’s exact shoulder bone structure and map out the entire procedure in detail.
“We’re able to customize the surgery based on the patient’s unique anatomy,” Dr. Keener says. “Then we use 3D-printed guides in the operating room to help place the implant exactly where it needs to go.”
This level of precision can lead to better range of motion, fewer complications, and longer-lasting results. Although this technology is still not used everywhere, it’s become the standard at Barnes-Jewish Hospital, helping more patients heal faster and feel better.
Custom implants for complex cases
Standard implants might not be an option for patients with significant bone loss, or in rare cases where a previous shoulder replacement has failed. That’s where custom-made implants come in.
“With 3D printing, we can now create implants that are designed to fit a person’s unique shoulder structure,” Dr. Keener says. “This gives us a way to help patients who might not have had any other options before.”
WashU Medicine orthopedic surgeons have extensive hands-on experience with custom implant surgeries, providing patients with advanced expertise and personalized care. These newer implants are built to work better in tough cases, helping more people get back to doing what they love.
Pyrocarbon: A smoother, longer-lasting shoulder option
One of the newest advances in shoulder replacement surgery is the use of a special material called pyrocarbon in certain procedures. This material is smoother than metal, gentle on the surrounding bone, and moves more like natural bone.
“Because it’s slick and not as hard as metal, pyrocarbon doesn’t wear down the socket as quickly,” Dr. Keener explains. “It helps protect the joint and surrounding bone over time and potentially allows very active patients to maintain their activity levels.”
“Partial shoulder replacements used to mean more stiffness and faster wear,” Dr. Keener says. “With pyrocarbon, patients are staying active longer, with less pain and fewer complications.”
WashU Medicine is leading a nationwide study, in collaboration with four orthopedic centers, comparing pyrocarbon to traditional materials, with early signs pointing to better long-term results.
Helping patients with shoulder instability
While WashU Medicine orthopedic surgeons treat a high volume of complex shoulder conditions, they are also focusing on shoulder instability, a condition where the shoulder slips out of place, often affecting younger, active patients.
“These surgeries can be life-changing,” Dr. Keener says. “Patients often recover their strength, stability, and confidence.” The team is experienced in challenging instability cases and welcomes patients looking for advanced surgical expertise.
A leader in orthopedic care
Barnes-Jewish Hospital and WashU Medicine are recognized as one of the top programs in the country for orthopedic surgery, not only for its skilled surgeons but also for its commitment to innovation and patient care. The team is on the forefront of developing new approaches, from advanced planning tools to unique surgical techniques for patients with severe joint damage.
“We have the technology and experience,” says Dr. Keener. “But most importantly, we have a commitment to doing what is best for each patient, and that is what drives all of this forward.”
