Donors who support the following Christian Hospital Foundation initiatives may be eligible to receive Neighborhood Assistance Program (NAP) tax credits.
Food Pharmacy Program
This initiative addresses food insecurity by providing nutritious food to patients diagnosed with congestive heart failure and diabetes. By ensuring access to healthy food, the program reduces hospital readmissions and emergency room revisits within 30 days of discharge, creating sustainable pathways to better health.
Medical Indigent Fund
The Medical Indigent Fund supplies underinsured and uninsured patients with life-saving medications, medical devices, and access to primary care. For example, the fund has enabled patients to receive wearable cardioverter defibrillators, which monitor and protect against sudden cardiac arrest.
Mobile Integrated Health Program
This innovative program brings healthcare directly to patients in their homes, addressing gaps in care and managing chronic conditions to prevent avoidable visits to the hospital.
