Step 3: Prepare for the Visit

Gather any necessary documentation, like athletic or school forms and vaccination records, and bring them to the appointment. Don’t forget glasses or contact lenses for the vision check. You and your child should be ready to answer questions about medical and personal health history, including previous injuries.

What should I bring to a school or sports physical exam?

If the school, camp, or league requiring the physical exam has provided paperwork, bring it to the appointment along with the following:

  • Immunization/vaccination records

  • All medications your child takes, as well as over-the-counter medications and supplements

  • Primary care provider’s contact information

  • Medical history records

  • Corrective lenses such as glasses or contact lenses

  • Have your child wear gym shoes or bring them to the appointment.

Does the $35 also apply to college athletics and students?

Many colleges and universities have specific requirements that require a more in-depth exam; therefore, the cost of a physical will be higher. The $35 applies to elementary and high school-aged kids.

When should a child get a sports or school physical?

BJC recommends getting a youth physical about 6 to 8 weeks before the activity, sports season or first day of school. 

 

What can I expect after a sport’s physical exam?

If the BJC provider determines that your child is safe to play in their sport, they will complete and sign the required sports physical form. Please have this available at the appointment as it includes information about your child’s current health, medical history and any conditions that might increase a risk of injury. The form also may include your child’s vaccination record. 

What is done for a sports or school physical?

If the BJC provider determines that your child is safe to play in their sport, they will complete and sign the required sports physical form. What if the provider discovers a problem during the youth physical?

If the BJC provider finds an issue such as an injury or condition that might indicate a problem, they may recommend another evaluation with the child’s pediatrician or family medicine provider.

What is the difference between a sports physical at a convenient care location and an annual physical with a pediatrician or family medicine provider?

Annual physicals are comprehensive evaluations of a child’s overall health and development, providing an opportunity for ongoing monitoring of developmental milestones and preventative care.
