If the school, camp, or league requiring the physical exam has provided paperwork, bring it to the appointment along with the following:

BJC recommends getting a youth physical about 6 to 8 weeks before the activity, sports season or first day of school.

If the BJC provider determines that your child is safe to play in their sport, they will complete and sign the required sports physical form. Please have this available at the appointment as it includes information about your child’s current health, medical history and any conditions that might increase a risk of injury. The form also may include your child’s vaccination record.
