I gave my 4-year-old a Pepcid 20mg by mistake. Do I have to take him to the emergency room?

Pepcid (famotidine) is a medication that is indicated for use in children. Not knowing the weight of your child, I can tell you the recommended dose is 2mg/kg (2.2lbs) per day up to a maximum of 40mg per day, divided into 2 doses. You do not need to take the child to the ER. Always keep medications out of the reach of children. Always double check all medications given to children.
