Kathryn “Katie” Maclean, MSW, LCSW

About Me

I have spent most of my professional career in the field of chronic disease management, working with the Alzheimer’s Association and the National Multiple Sclerosis Society. I am currently licensed in Missouri and am pursuing licensure at the clinical social work level in Illinois as well.

My Qualifications

I am a licensed clinical social worker in St. Louis, Missouri since 2011. My research publications include contributions to the American Medical Associations Physician’s Guide to Counseling and Assessing Older Drivers and A Day in the Life of A Gerontological Social Worker. I attended Saint Louis University where I was selected as the Community Concentration Social Worker of the Year. During my training I was selected to participate in the John A. Hartford and The Social Work Leadership Institute of the New York Academy of Medicine’s Partnership Program, and was awarded the 2006 AARP Diversity and Aging Fellowship.

What Inspires Me

I enjoy working with older adults, especially those affected by all types of dementia, and their caregivers to help the seniors have a high quality of life.

The Post-Acute Care Team

BJC Accountable Care Organization’s (ACO) post-acute care team provides continuity of care as patients transition from the hospital to post-acute care/skilled nursing facility and then back to their home setting.

Amrita Singh, MD, is part of the post-acute care program of BJC Accountable Care Organization (ACO). Dr. Singh is a highly trained and experienced geriatrician who provides easy access for patients during this critical time. She received her medical degree from Government Medical College. She completed her internship and residency at St. Joseph Hospital in Chicago. She also completed a fellowship in geriatric medicine at Rush University Medical Center in Chicago.

Gina Uzendu, NP, has been practicing since 2007. She attended nursing school at Maryville University in St. Louis, MO and completed a preceptorship with Veteran’s Affairs Hospital.

Mweemba Chipepo, NP, is a board-certified Adult Nurse Practitioner. He received his Master’s of Science in Nursing from Goldfarb School of Nursing at Barnes-Jewish College. Mweemba has a unique passion for serving the elderly and provides compassionate care to all his patients.

Lara Kerwin, PharmD, BCACP, is a board-certified, ambulatory care pharmacist. She received her Doctor of Pharmacy degree from the University of Missouri-Kansas City School of Pharmacy and completed a two-year residency at the University of Minnesota College of Pharmacy. Her areas of expertise include patient-centered, pharmacist-provided care and leadership.

Hello, my name is Adam and I work as a care partner to support patients transitioning from hospital to home by offering them assistance with medications, follow up appointments, and therapies. I also assist patients with chronic health care needs who may not have had a recent hospitalization, but may benefit from an additional layer of nursing support. I work closely with patients’ physicians for continuity of care and timely management of health care issues.

The first thing people usually ask me is if I was named after Disney’s The Little Mermaid, Ariel; the answer is no. When I was born my mother was on a Shakespeare kick and named me after a sprite from Shakespeare’s play The Tempest. I am a nature enthusiast and I love hiking, camping, kayaking and exploring. One of my biggest travel dreams is to explore the Amazon River with my husband.
