About me

Lara Kerwin, PharmD, BCACP, is a board-certified, ambulatory care pharmacist.

My qualifications

I received my Doctor of Pharmacy degree from the University of Missouri-Kansas City School of Pharmacy. Next, I completed a two-year residency at the University of Minnesota College of Pharmacy. My areas of expertise include patient-centered, pharmacist-provided care and leadership.

What inspires me

I am inspired by seeing people learn, improve their confidence, and achieve their goals. I am also inspired to do my best by working alongside hardworking, compassionate team members.

What I would like my patients to know

Every day, I work to prevent and solve medicine problems. I do this by:

  • Listening and keeping in mind your needs, questions, and goals.
  • Making sure all medicines are needed, working well, safe, easy to take, and affordable.

Feel free to ask me questions. I love to make medicines “make sense” so you can make good health decisions.

I can update your care team about your concerns. I provide health recommendations and resources to improve your overall quality of life.

The Post-Acute Care Team


BJC Accountable Care Organization’s (ACO) post-acute care team provides continuity of care as patients transition from the hospital to post-acute care/skilled nursing facility and then back to their home setting.

Amrita Singh, MD

Amrita Singh, MD, is part of the post-acute care program of BJC Accountable Care Organization (ACO). Dr. Singh is a highly trained and experienced geriatrician who provides easy access for patients during this critical time. She received her medical degree from Government Medical College. She completed her internship and residency at St. Joseph Hospital in Chicago. She also completed a fellowship in geriatric medicine at Rush University Medical Center in Chicago.

Gina Uzendu, NP, has been practicing since 2007. She attended nursing school at Maryville University in St. Louis, MO and completed a preceptorship with Veteran’s Affairs Hospital.

Mweemba Chipepo, NP

Mweemba Chipepo, NP, is a board-certified Adult Nurse Practitioner. He received his Master’s of Science in Nursing from Goldfarb School of Nursing at Barnes-Jewish College. Mweemba has a unique passion for serving the elderly and provides compassionate care to all his patients.

Hello, my name is Adam and I work as a care partner to support patients transitioning from hospital to home by offering them assistance with medications, follow up appointments, and therapies. I also assist patients with chronic health care needs who may not have had a recent hospitalization, but may benefit from an additional layer of nursing support. I work closely with patients’ physicians for continuity of care and timely management of health care issues.
