The Post-Acute Care Team


BJC Accountable Care Organization’s (ACO) post-acute care team provides continuity of care as patients transition from the hospital to post-acute care/skilled nursing facility and then back to their home setting.

Lara Kerwin, PharmD, clinical pharmacist, BJC Medical Group

Lara Kerwin, PharmD, is a registered pharmacist through the Missouri State Board of Pharmacy with a Medication Therapy Services (MTS) certification. She received her Doctor of Pharmacy degree from the University of Missouri – Kansas City School of Pharmacy in Kansas City, MO and then completed a two-year post-doctoral residency program at the University of Minnesota College of Pharmacy. During residency, she spent time specializing in ambulatory care pharmacy practice, professional advocacy, clinical pharmacy service development and expansion, teaching, and implementing quality improvement initiatives in health care.

Mweemba Chipepo, NP

Mweemba Chipepo, NP, is a board-certified Adult Nurse Practitioner. He received his Master’s of Science in Nursing from Goldfarb School of Nursing at Barnes-Jewish College. Mweemba has a unique passion for serving the elderly and provides compassionate care to all his patients.

Gina Uzendu, NP

Gina Uzendu, NP, has been practicing since 2007. She attended nursing school at Maryville University in St. Louis, MO and completed a preceptorship with Veteran’s Affairs Hospital.

Bethany Miller, RN

My experience as a registered nurse has been in cardiology, inpatient case management and management for the past 6 years. I believe I share a common goal with the team and that is to meet our patients’ needs by delivering high quality, collaborative care. 

The ACO Assistant Nurse Managers roles and responsibilities are to assist the program director in the day to day operations of the team. The ACO team consists of registered nurses, social workers and care coaches that make telephonic outreach to patients that have recently discharged from an acute care setting, as well as patients that need longer term disease management and support.
