I am a patient advocate and reach out to patients that have been discharged from the emergency department and/or the hospital. I provide health education, involving their medications and answer any questions they may have, in regards to the recent visit. I also will reach out to any providers that need to urgently reach the patient, along with scheduling. My main objective in this job/position is to care for my patients and not let any concerns or questions go unanswered.
I have over 12 years working in the medical field in physician offices, assisted living, nursing homes, and memory care units. Through this experience I understand it is sometimes difficult for patients to talk about health problems. I encourage them to be open about their health to me and all the providers involved in their care so we can keep them at home where they are comfortable and with their loved ones. Patients have access to my work line at any time and I will be happy to speak with them about their care. I try my best to have every question answered in a timely fashion, because your worry is also mine.
