Bring your current insurance card(s) each time you visit our office. Many plans will not allow us to see you unless you present your card at the time of your appointment. We will accept your co-payment when you check in. Our office accepts cash and checks; and Visa, MasterCard, American Express and Discover credit cards, as well as debit cards displaying one of the credit card logos. We cannot accept ATM debit cards without logos.

When We Don't Participate in Your Insurance Plan
If you are a member of a plan in which our physicians do not participate, you may be able to use "out of network" benefits. Check with your plan administrator for guidelines. Because we do not participate in these plans, we are unable to file claims to your insurance carrier, request referrals, or obtain authorizations for tests or medications.

As an alternative to "out of network" benefits, you may receive a discount for payment in full at time of service.
