Clients without insurance coverage must use BJC HealthCare laboratory draw sites. Clients with Medicaid or other insurance may choose BJC HealthCare laboratory draw sites.
Laboratory Orders and Requisition Forms
The laboratory requisition form drives the laboratory process and must be properly completed and provided to the client to access laboratory services.
There are separate requisition forms for adults, and children and adolescents. The form for adults is white and reads, "Barnes-Jewish Hospital" at the top. The form for children and adolescents is green and says, "St. Louis Children's Hospital" at the top. Both are four-part forms. The top three copies go with the client to the laboratory. The gold copy may used for your files. The reverse side of the requisition lists available profiles. Place a checkmark in the box for each test requested.
Identification and billing information must be complete
- Mark appropriate line for client's BJCBH & BHP site location
- Client information -- name, address, date of birth, gender, social security number and phone number
- ICD 9 code
- Billing information
- For any insurance, mark "insurance"
- If the patient has no coverage, mark "account"
- Billing to Medicare
- Billing to Medicaid
- Insurance information for insurance companies other than Medicaid and Medicare
- Ordering physician's signature and contact
Standing Orders
The outpatient labs at Barnes-Jewish Hospital and BJCBH &BHP sites accept standing laboratory orders -- weekly WBC for Clozaril, urine drug screens or pregnancy screens.
Laboratory Results
Results of laboratory procedures performed at BJC HealthCare facilities are transmitted to BJCBH & BHP every day for review and inclusion in clinical records. BJCBH & BHP nursing staff review lab results and contact patients' physicians by phone, if outcomes fall outside normal ranges. The laboratory may be contacted directly for results. Results are provided by phone, fax or mail.
