Guidelines

  • Be proactive
  • Consider advance directives for hospitalized patients and early in a chronic illness
  • When predicting prognosis, give a range of possibilities -- based on recent evidence -- not a specific date
  • Determine a patient's "quality of life," which helps setting care goals
  • Regularly re-discuss and evaluate health-care goals
  • Participate in decision-making, treatment options and patient preferences
  • Consider appropriate environment for the delivery of palliative care
  • Discuss code status
