Continuum of Care

Wellness Initiatives
BJC HealthCare's purpose to improve health is carried out through the system hospitals' wellness initiatives. Throughout the communities it serves, BJC and its hospitals offer hundreds of health lectures and screenings. The system sponsors an extensive senior health membership program and operates the region's largest school outreach health initiative. BJC's affiliated hospitals are out in communities providing mobile mammography, physician referral, immunization programs, and health care and translation services for immigrants and refugees.

Other innovative community programs address crucial social issues, such as domestic violence, teen pregnancy, infant mortality, gun safety and at-risk youth. BJC's Spiritual Care Council offers spiritual awareness programs and accredited clinical pastoral education. In addition to working closely with local, county and state health officials, BJC HealthCare partners with schools, businesses, community and religious groups, and law enforcement agencies to improve health at every opportunity.


Ambulatory Care
Outpatient Services
As health care continues to move beyond the traditional hospital walls, BJC affiliated hospitals' outpatient services have expanded. More than 40 communities are home to BJC HealthCare comprehensive health centers, physician offices, health clinics, fitness and rehabilitation centers, and urgent care facilities.

Home Care Services
Whether preventing or treating illness at home, BJC Home Care Services complements our system's continuum of care through a comprehensive program of home care and hospice services.

BJC Home Care Services serves thousands of patients in more than 25 counties in Missouri and Illinois through its eight licensed agencies. Services include skilled nursing visits, private services, rehabilitation, adult and pediatric hospice, home infusion therapy, home medical equipment, and respiratory services.

Specialty home care programs also are available, including cardiac home care, maternal-child/pediatric services, adult and pediatric diabetes management, and orthopedic care.

Transportation Services
Six ambulance services are affiliated with BJC HealthCare. And St. Louis Children's Hospital supports a mobile intensive care unit for critically ill children.

Acute Care
Inpatient Services
BJC HealthCare has 13 affiliate hospitals throughout the St. Louis and mid-Missouri regions. Two of the hospitals -- Barnes-Jewish Hospital and St. Louis Children's Hospital -- consistently rank among the best in the country. These two hospitals serve as teaching facilities for Washington University School of Medicine, which is nationally renowned for education and research.

Rehabilitative Services
Comprehensive fitness rehabilitation centers provide outpatient physical therapy as well as cardiac and pulmonary rehabilitation. Work-related injuries are addressed by BJC Corporate Health Services, which also provides companies with pre-placement physicals and screenings, comprehensive ergonomics services and on-site wellness programs.

Subacute Care
Subacute care is post-hospital, often post-surgery, care that is more intensive than skilled nursing care, but less intensive than hospital care. Subacute care is provided by BJC Home Care Services for patients who continue to need intravenous therapy, ventilator or tracheostomy care, or treatment of extensive wounds. It is also for patients who require extensive rehabilitation but have complicating medical conditions.

Subacute care covers frequent and around-the-clock monitoring by a licensed nurse, as well as frequent review of each patient's regimen of care by their physician, often one or more times a week.

Long-Term Care
For intensive rehabilitation and extended care, there are skilled nursing facilities and a retirement community within BJC HealthCare where older adults have easy access to medical care. For terminally ill patients, BJC Home Care Services operates four non-profit, Medicare/Medicaid-certified hospice programs. These hospices offer care and support to both patients and their families, specializing in pain relief and offering emotional and spiritual counseling so that patients may die with dignity.

Hospice
BJC Hospice offers a multidisciplinary approach, holistic in nature, to people with a life-limiting illness. The care, sensitive to the emotional and spiritual needs of adult and pediatric patients, is centered on the patient and family, and provides continuity of care facilitated by a highly skilled team of professionals.

Hospice focuses on the needs of the patient, and family. The BJC Hospice team works within your home, helping manage discomfort, coordinates home medical equipment and pharmacy requirements, and provides counsel on spiritual and emotional needs. Volunteers assist with personal care, errands and household tasks.
